How to Refer to Us
At Associated Audiologists, we work with a large network of referring physicians and primary care providers in the greater Kansas City, Leavenworth, and Manhattan communities. Currently, Medicare beneficiaries must obtain a physician referral prior to seeing an audiologist in order to have those services reimbursed by Medicare. To assist with your referral, please download and complete the Patient Referral Form below and fax to the appropriate office.
Orders should read as follows based upon the service being requested:
Diagnostic hearing evaluation to determine hearing loss and/or site of lesion
OR
Diagnostic hearing evaluation for sudden hearing loss
OR
Evaluation and treatment of dizziness/vertigo/imbalance
OR
Evaluation middle ear pathology
OR
Evaluation and management of tinnitus
OR
Rule out retro-cochlear pathology
Please complete the Patient Referral Form and fax to the appropriate number.
